Appointments and Consultation 

We request the minimum level of personal details in order to carry out treatments in a safe and appropriate manner - these details will include (but are not limited to) your name, address, date of birth, medical history and current physical and mental health (including but not limited to prescribed medication, allergies, injuries or conditions including pregnancy or chronic illness). 

Your details and appointment notes will be stored for a period of 7 years to ensure we can continue to provide a safe and appropriate treatment for you and exceed our level of service. Your personal details are also required in order to contact a next of kin or health professional in case of an emergency or urgent care is required.

• Information provided when participating in discussion boards or other social media platforms (including but not limited to Facebook, LinkedIn and Instagram)

Information we collect about you

When visiting our website we may automatically collect the following information:

 IP (Internet Protocol) address used to connect to your computer to the internet, browser type and version, login information, time zone setting, browser plugin types and versions operating system and platform, URL( Uniform Resource Locators) clickstream to, through and from our website (this includes date and time) products and services you viewed or searched for, download errors, page response times, length of visits to webpages on our websites, page interactions (clicks, mouse-overs and scrolling) and methods used to browse away from the page and any phone number used to call our contact number, 

Client record keeping

Your data will be stored in a highly secured locked filing storage cabinet, which will only be accessible to Relax and Rebalance Holistic Therapies employees. We will hold your personal details including appointment notes for a period of 7 years unless requested otherwise.

Unfortunately, the transmission of information via the internet is not completely secure. We will do our best to protect your personal data but cannot guarantee the security of your data transmitted to our site; any transmission is at your own risk. Once we have received your information, we will use strict procedures and security features to try to prevent unauthorised access.
